I confirm my dog is desexed and vaccinated (C5 minimum). If they are a puppy, confirm they will be desexed at the earliest possibility or by 1st birthday. A copy of your vaccinations papers will need to be provided prior to assessment.*

I understand that a 1.4% surcharge is payable on all credit card payments. *

I understand that my dog/s may get a little dirty whilst at doggie daycare.

I understand that there is a 24hr cancellation policy and all fees must be paid if less than 24hrs notice is provided.
